Quality Assurance/Quality Control in Translation & Localization: How do you do it?

I am working on a paper for an upcoming academic colloquium dealing with a new approach to quality assurance and quality control in the field(s) of translation and localization.

As part of my research, I’m very interested in what you might have to add: what are your views and experiences in QA and QC in your respective field(s)? How do you do it? If you don’t have QA or QC procedures in place: why not? What have you found to be best practices, poor practices, protocols which work, which have failed, etc? I’m equally interested in comments relating to editing or any related field. I welcome any insight you have to contribute.

Academic disclosure: this paper does not count toward my degree in any way and is purely for personal interest and general advancement in the field. I will of course credit and appropriately cite any information you provide—and which I use--in the final paper; if you wish to remain anonymous, please mention this in your reply. As humans are not subjects of this research, ethics approval is not required and was not obtained.
